Event description
Employee's leg partially amputated when struck by load
Investigation abstract
Employee #1 was guiding a load of formed steel plate that was being moved by an overhead crane. Twelve 12 in. by 12 in. by 10 ft 8 in. pieces of 3/8 in. plate f ormed into channel shape had been bundled together and rigged with an 18 ft by 4 in. web sling in a basket hitch. The sling weight capacity was 11,400 lb. The b undle weighed 5,748 lb. When the load became off balance, Employee #1 tried to h old one end down, but he was lifted off the floor and let go. The load slid ahea d and a sharp edge on one of the pieces of steel cut the web sling and dropped t he load. Employee #1's leg was partially amputated at the ankle and his foot was completely crushed.
