Event description
Employee injured in 30 ft fall from scaffold
Investigation abstract
Employee #1 was working on a concrete form scaffold when the concrete pump truck hose kinked and whipped. The hose hit Employee #1 and knocked him off the scaff old. He fell 30 ft to the ground. Employee #1 was treated on the scene by Hercul es Medics, then transported to University of Utah Hospital. Employee #1 was inco herent five days after the accident.
