Event description
Employee killed when water tanker rolls over
Investigation abstract
Employee #1 was operating a Caterpillar 641 water tanker that was about half ful l. As he made a turn from one access road to another, the left rear wheel rode u p on an approximately 2 ft high beam. This caused the water inside the tanker to surge forward and to the right. The combination of this and the slope of the ro ad caused the tanker to roll over 1 1/4 times, ejecting Employee #1. He was kill ed when crushed under the windshield and operating area.
