Employee injured when struck by falling boiler spacer
Investigation abstract
Employee #1 was the ground man for a skyclimber work operation when a boiler tub e spacer broke loose from the top. The spacer fell, struck a protruding arch, an d then bounced outward into a cavity. The spacer then struck the opposing wall, bounced again, struck Employee #1 on his hard hat, and then on his left hand, br eaking the index finger and cutting his glove's outside palm. His right hand was not injured. The employee was in the ash collecting area.
