Event description
Employee's finger amputated in machine point of operation
Investigation abstract
Employee #1, a draw bench lead operator, was adjusting the west side of a machin e while a coworker was operating the east side of the machine. Both operators, w ho stood on either side of the machine, had a control panel. While the coworker was operating his side of the machine, Employee #1's finger became caught in the machine's moving parts (jaws and mandrel) and was amputated. The machine's poin t of operation guarding was inadequate for both operators.
