Employee killed when run over by trolley wheel
Investigation abstract
Between 1:10 and 1:15 p.m. on March 19, 1986, Employee #1 arrived to adjust a le at he could adjust the switch while the operator was gone. The operator returned to the trolley and, not knowing that Employee #1 was in front, started the trol ley forward. The trolley ran over Employee #1 and killed him. vel limit switch on the north transfer trolley as he had been assigned. The trol ley operator told him that because they were running short on truck bodies, he h ad to work during his break and there was not time for him to adjust the switch. Employee #1 suggested that he come back at 2:30 p.m. and the operator said that would be great. Employee #1 departed and the operator continued to operate the trolley.
