CAUGHT BY, OR FELL INTO, UNGUARDED SWING SAW

Investigation abstract

EMPLOYEE #1, WORKING AS A CUT-OFF SAW OPERATOR, WAS USING A SWING SAW THAT HAD A LIMIT CHAIN AND NO BLADE GUARD. HE EITHER PULLED THE SAW INTO HIS BODY OR FELL INTO IT. THE SAW HAD AN UNGUARDED 30 INCH SAW BLADE. EMPLOYEE #1'S LEFT HAND WAS AMPUTATED AND HIS BODY WAS CUT FROM HIS NOSE TO HIS CROTCH. HE DIED IMMEDIATELY . APPARENTLY THERE WERE NO WITNESSES TO THE ACCIDENT.
