Event description
Five employees killed when motor ignites
Investigation abstract
Employees #1 through #5 were removing a core from a Peacekeeper stage 1 motor. T he employees had popped the core more than 10 inches, then apparently went into the building to finish removing the core from the motor. It appears that the emp loyees were using the hydraulic system at the facility, not at the remote contro l room, to remove the core, causing the motor to ignite. The burning happened so fast that the employees were unable to escape. Employees #1 through #5 were kil led. Further investigation indicates that the employees' operating procedure was not in compliance with the shop traveler. Other crews were using the same proce dures.
