CONSTRUCTION EMPLOYEE STRUCK AND KILLED BY FALLING PIPES

Investigation abstract

AT APPROXIMATELY 10 AM ON DECEMBER 5, 1986, EMPLOYEE #1 WAS HELPING UNLOAD SEWER WAS COVERED BY THE SEWER PIPES AND SUFFERED FATAL INJURIES. HE DIED AT THE SCENE . PIPE FROM A FLATBED TRAILER. THE 8 INCH DIAMETER BY 12 FEET LONG PVC SEWER PIPE WAS STACKED 5 HIGH AND 5 ACROSS, SEPARATED BY PLASTIC SPACERS. THE 25 PIPES IN EACH BUNDLE WERE HELD TOGETHER BY 1/2 INCH METAL STRAPS ON EACH END. TWO BUNDLES WERE STACKED, ONE ATOP THE OTHER, HELD TOGETHER BY ONE 1/4 INCH METAL STRAP ARO UND THE MIDDLE OF BOTH BUNDLES. EMPLOYEE #1 WAS STANDING ON THE EDGE OF THE TRAI LER, CUTTING THE 1 1/4 INCH STRAPS HOLDING THE TWO BUNDLES TOGETHER. THE METAL S TRAPS AT THE ENDS OF THE TOP BUNDLE FAILED, CAUSING 25 SEWER PIPES, WEIGHING 125 POUNDS APIECE, TO ROLL OFF THE TRAILER AND KNOCK EMPLOYEE #1 TO THE GROUND. HE
