Investigation abstract

EMPLOYEE #1, A PERSONNEL HOIST OPERATOR, DROPPED A PACK OF CIGARETTES UNDER THE HOIST AND ATTEMPTED TO RETRIEVE THEM. WHILE HE WAS UNDER THE HOIST THE COUNTERWE IGHT DESCENDED AND STRUCK HIM, CRUSHING HIS CHEST AND KILLING HIM.
