Employee killed in fall from stock shelf

Investigation abstract

While looking for misplaced merchandise, Employee #1 fell 87 in. from the top of a stock shelf. She struck her head on the floor and died of a subdural hematoma . Step stools are provided and it is required that they be used. For some reason , Employee #1 did not use one.
