Investigation abstract
FLAMMABLE VAPORS WERE EMITTED WHEN EMPLOYEES WERE PULLING A DRILL STEM TEST. THE VAPORS WERE IGNITED BY AN UNKNOWN SOURCE, CAUSING THE RIG TO CATCH FIRE AND BUR NING EMPLOYEES #1 AND #2.
