EMPLOYEE KILLED IN FALL FROM TRUCK BED
Investigation abstract
EMPLOYEE #1 AND A TRUCK DRIVER DROVE TO A JOB SITE TO DELIVER SOME 55 GALLON DRU MS OF OIL AND TO PICK UP SOME EMPTY DRUMS. THEY UNLOADED THE FULL DRUMS. EMPLOYE E #1 THEN CLIMBED ONTO THE BACK OF THE FLAT BED TRUCK AND INSTRUCTED THE DRIVER TO DRIVE TO THE AREA WHERE THE EMPTY DRUMS WERE STORED. WHEN THE TRUCK BEGAN TO MOVE EMPLOYEE #1 FELL OFF THE BACK AND STRUCK HIS HEAD ON A CONCRETE SLAB. HE WA S KILLED. THE TRUCK BED WAS 45 1/2 INCHES ABOVE THE GROUND.
