Employee killed when struck by mast

Investigation abstract

A mechanic, a welder, and the machine operator were removing the final drive ass embly from a mast on a drilling machine. The mast fell on Employee #1, killing h im. The employees had used only four bolts to hold the mast and drive assembly; there were supposed to be ten bolts holding the units together.
