Investigation abstract
At approximately 9:45 p.m. on October 26, 1988, Employee #1, a laborer for Corre ct Maintenance Corporation of Portage, IN, was cleaning heat exchangers on build ing 18C at A.E. Staley Manufacturing Company in Lafayette, IN, when he fell 43 f t to the ground. Employee #1 died of massive head injuries on October 30, 1988.
