Employee killed when struck by falling conveyor guard
Investigation abstract
At approximately 6:30 a.m. on August 8, 1988, Employee #1 was supervising the cl ean-up of asphalt that had fallen to the ground from a bin and conveyor malfunct ion. A 28 lb guard, which had been broken from the conveyor system, fell approxi mately 40 ft and struck Employee #1 on the head, killing him.
