Investigation abstract

AN EMPLOYEE WAS CAULKING AN ENCLOSURE THAT HAD BEEN BUILT AROUND A TRANSFORMER. HE WAS PERFORMING A TASK THAT WAS NOT NORMALLY ASSIGNED TO HIM. HE WAS ELECTROCU TED WHEN HE CONTACTED ENERGIZED TERMINALS ON THE TRANSFORMER.
