Event description

Three employees injured in scaffold collapse

Investigation abstract

Two employees were on a 15 ft scaffold and one employee was on the floor during demolition at building 783. A heat shield fell from a beam and struck the scaffo ld, causing it to collapse and Employee #1 to fall to the floor, where his leg w as impaled by a scaffold pin. Employee #2 fell astride a rail of the scaffold an d fractured his tailbone. Both Employees #1 and #2 required hospitalization. Emp loyee #3, who was on the floor, bruised his leg while running.
