Employee killed in fall from scaffold buck
Investigation abstract
Employee #1 was installing metal strips and wire on the side of a multi-storied building while on the sixth or seventh level of a nine buck high tubular welded scaffold with guardrails. The scaffold bucks were each 77 in. high. For some rea son, Employee #1 fell 35 to 40 ft from the sixth or seventh level buck and died of internal injuries.
