Employee killed when struck by truck box
Investigation abstract
Employee #1, an assistant on a refuse removal truck, opened the truck's tailgate and then stood between two trucks. The driver of the second truck blew his horn to warn Employee #1 to get out of the way. The second truck's box tipped to the side, striking and killing the employee.
