Event description

Employee killed when crushed by steel coil

Investigation abstract

At approximately 5:35 p.m. on August 26, 1988, Employee #1 was told by his forem a cause. Employee #1 was authorized to ride the forklift truck, but did not rece ive forklift operator training from the company. an, the plant superintendent, to move a seven-piece steel coil weighing 5,000 lb with a Towmotor forklift truck to the banding area. The foreman then left the e mployee to work alone. After Employee #1 lifted the cold form, oil lubricated st eel coils off the floor, he apparently got off the forklift truck and walked to the front of the load to see if it was properly centered. The employee had left the load engaging means at a downward angle and the steel coils slid off the eng aging means and struck him in the head, crushing his head. Employee #1 was kille d.
