Event description

Employee's fingertip amputated in clevis pin hole

Investigation abstract

On January 26, 1989, Employee #1, a maintenance mechanic, was inside the housing of a machine replacing a broken clevis. A jaw assembly that was attached to a c rane moved, causing the employee's glove to become caught in the clevis pin hole . His left fingertip was partially amputated.
