Employee's fingertip amputated in onion dispenser
Investigation abstract
Employee #1 was operating an onion dispenser on a production line where frozen T e and was amputated. V dinners are manufactured. She was using a plastic handheld scoop to move pearl onions out of a box and into the dispenser. The bottom plate of the dispenser h ad six holes through which the onions dropped onto a dinner plate that was passi ng on a conveyor line below. The bottom plate revolves continuously and a shear point is created by a metal bar that holds the onions back so that they drop dow n into the holes on the plastic disc plate. The employee had her right hand in t he dispenser breaking up some frozen onions that were stuck together when the ti p of her index finger went down into one of the holes on the revolving disc plat
