EMPLOYEE STRUCK BY OBJECT FALLING OFF LIFT TRUCK FORKS
Investigation abstract
A fork-lift operator left the operator's seat and walked to the front of the for k-lift. The load of three large ductile iron pipes on the raised forks was unsta ble and the front pipe fell on the employee. The employee may have brushed the e nd of the pipe dislodging it. The 865 pound pipe crushed the employee's left sho ulder and skull which resulted in massive head trauma killing the employee.
