EMPLOYEE INJURED IN FALL FROM LADDER TO CONCRETE FLOOR
Investigation abstract
At approximately 9:00 a.m. on May 2, 1989, Employee #1 was working alone on a 10 9, the employee was still hospitalized. Lack of employee training and failure to use personal protective equipment contributed to this accident. -10 A-frame extension ladder. He had been instructed to remove and replace condu it that was run inside a concrete block wall. An opening for ductwork had been m ade and was large enough for a man to climb through. The bottom of the opening w as 14 ft 3.5 in. above the floor. The conduit would have been accessible through the duct opening. No there were no witnesses; however, to remove the conduit (i nside wall) Employee #1 would have been working at the 14 ft 3.5 in. location. T he employee fell from the 10-10 A-frame extension ladder or from the wall openin g onto concrete floor which resulted in severe head injuries. As of July 17, 198
