Employee's fingers amputated in unguarded saw blade

Investigation abstract

Employee #1 was cutting a 3/8 in. plywood sheet on a radial arm saw. He was turn ing back to cut another piece when his right hand struck against the back of the unguarded blade and three of his fingers were amputated.
