Employee killed when crushed in cheese blender
Investigation abstract
Employee #1, a sanitation worker, was cleaning a cheese blender while the blades were rotating. When he reached into the machine to wipe cheese from the blades, he became caught and was pulled into the blender. He sustained a crushed chest and fractured back, and was killed.
