EMPLOYEE WAS CAUGHT BETWEEN AND CRUSHED BY MOVING OBJECT
Investigation abstract
At 1:00 p.m. on March 21, 1988, Employee #1 hooked a load to a side boom mounted on a caterpillar tractor. He jumped onto the moving tractor, or ran alongside o f it, slipped and fell on the treads. He was crushed to death between the tracto r treads and side-boom counterweight.
