EMPLOYEE KILLED IN FALL DOWN ELEVATOR SHAFT
Investigation abstract
Sometime between the evening of January 5, 1988, and the morning of January 6, 1 988, an employee fell from an 11th-floor elevator hatchway to a concrete surface at the bottom of am elevator shaft. The employee was found at the bottom of the elevator shaft at approximately 7:45 am on January 6, 1988. The fall distance w as approximately 110 feet to 120 feet. The elevator car was found stuck on the 1 2th floor and the hatchway door was found open on the 11th floor. The reason the employee was on the 11th floor is unknown.
