Event description

CONSTRUCTION EMPLOYEE KILLED WHEN STRUCK BY CONCRETE TRUCK

Investigation abstract

EMPLOYEE #1 WAS AT THE REAR OF A STATIONARY CONCRETE TRUCK OPERATING THE CHUTE T O FILL AN ABUTMENT WALL FORM. AT THE SAME TIME A SECOND TRUCK ARRIVED ON THE SIT E, DOWN THE HILL TO THE RIDGE, NOT SLOWING OR STOPPING. THIS SECOND TRUCK STRUCK THE FIRST TRUCK AND KNOCKED IT INTO THE CREEK, KNOCKED DOWN FORMS, AND KNOCKED EMPLOYEE #1 DOWN THE BANK. EMPLOYEE #1'S SKULL WAS STRUCK AND CRUSHED BY ONE OF THE TRUCK'S REAR WHEELS. THE OTHER TWO EMPLOYEES, WERE STANDING ON THE WALL FORM , WERE KNOCKED OR JUMPED INTO THE CREEK BED. EMPLOYEE #1 WAS KILLED. EMPLOYEE #2 HAD A BRUISED FOOT (LEFT HEEL). EMPLOYEE #3 HAD BRUISES AND ABRASIONS TO HIS BA CK.
