Event description

Employee dies after falling through roof panel

Investigation abstract

At approximately 10:15 a.m., Employee #1, a 29-year-old carpenter, and a coworke r were installing panelized roof sections between grid lines D and E and 1 and 2 . The employee was walking toward the open side of the section to adjust a truss that they had unbolted. The panel gave way and he fell approximately 29 ft to a concrete floor, sustaining severe head and internal injuries. Employee #1 was t ransported to St. Joseph's Barrow Neurological Institute, where he died at appro ximately 12:30 p.m. on July 8, 1988. At the time of the accident, he had been em ployed by Weyerhauser Building Systems for approximately 10 months.
