Employee's arm fractured in unguarded point of operation
Investigation abstract
At 8:07 p.m. on September 2, 1987, Employee #1 was attempting to retrieve a card that had been missed by the second pick-up. He turned off stationary line one u sing a STOP switch and had reached into the point of operation when the machine apparently restarted or jogged. His right arm became caught between braces and t wo pick-ups on station one, and it took five minutes to free him from the machin e. Employee #1 fractured his arm in three places and was hospitalized. The accid ent resulted from failure to guard the machine's point of operation.
