Event description

Employee killed when crushed by falling particle board

Investigation abstract

Employee #1 was standing 104 in. above the ground atop a stack of 4 ft by 8 ft s heets of plywood, counting the loose sheets next to him. At the same time, a for klift was getting material to be counted from the adjacent aisle. A 2,800 lb, 40 sheet unit of particle board, 25 in. by 97 in., was jarred loose by the action of the forklift and fell onto the employee's upper torso. He was crushed and kil led. The material then broke apart and fell to the ground, with Employee #1 fall ing on top of it.
