WELDER INJURED IN FALL FROM LADDER

Investigation abstract

ON NOVEMBER 3, 1987, EMPLOYEE #1, A WELDER, DESCENDED A FIXED LADDER TO PICK UP SOME WARM FLUX FOR HIS SUBMERGED ARC WELDING MANIPULATOR. HE THEN FILLED A 5 GAL LON PAIL WITH ABOUT 45 POUNDS OF WARM FLUX AND PROCEEDED BACK UP THE LADDER TOWA RD THE ELEVATED WELDING PLATFORM. ABOUT HALFWAY UP THE LADDER (6 TO 8 FEET) HE L OST HIS GRIP ON THE HANDRAIL AND FELL TO THE GROUND. HIS HEAD STRUCK THE FLOOR, RESULTING IN A FRACTURED SKULL AND RELATED TRAUMA.
