On or about 05/19/2015, a utility crew was pulling two sections of a utility pole together with come-a-longs. Each section of the pole has rigging plates attached. One of the rigging plates broke loose and struck the employee on the back resulting in a ruptured spleen that required inpatient care and surgery.
