On May 21, 2015, at approximately 9:30 AM, an employee and coworker were using a forklift and floor jack to remove a hydraulic drive motor from the distribution buggy for repairs. The motor slipped from the floor jack and crushed the employee's thumb, requiring hospitalization and surgery.
