An employee hooked an extension ladder onto a main overhead cable line and climbed up the ladder. His task was to replace a secondary cable going from the main cable over the sidewalk to a house. When he cut the old cable, which had tension in it, the cable swung, hit the ladder, and knocked the employee off the ladder to the sidewalk 18 feet below. The employee was wearing a safety belt and lanyard. The lanyard was not attached to the main cable at the time of the incident.
