Final narrative
On or about 6/11/2015, at 8:55 a.m., an employee working with a roof cutter attempted to seal a hole that was located on the inside panel of the machine. The employee reached into the machine and his glove got caught in the blade, amputating his ring fingertip and lacerating his middle finger. The machine was running at the time of the incident.
