Final narrative
An employee was doing a preliminary leak test on a polyethylene water pipe before backfilling the pipe. They were pressure testing the joint when the joint came loose in front of the end cap. The pressure was up to 140 PSI when the pressure released. The pressure release and/or possible flying objects or debris injured the employee. The employee sustained fractures to their jaw on both sides, and their right tibia. They also sustained tooth damage.
