An employee was clearing a clogged dust collector pipe. The employee was standing on a home-made platform that had been elevated approximately 10 feet in the air by a forklift. The platform measured approximately 12-feet long by 42-inches wide. The employee stepped off to the side of the platform where the platform was not supported by the forks, and the platform tipped from the forks. The employee fell approximately 10 feet to the ground below. The employee sustained nine broken ribs and a punctured lung.
