An employee climbed a fixed ladder to gain access to a roof. When he approached the top of the ladder, he opened a hinged lid and proceeded to climb up the ladder. The hinge failed and the lid struck the employee in the head causing him to fall approximately six feet to the ground below. The employee received a laceration to the face, contusions and bruising to the right shoulder, elbow and knee, and fractured his right ankle. The employee was transported to a local hospital where he was treated and released.
