On or about 7/22/2015, at approximately 5:15 p.m., an employee was using a shear saw to cut fiberglass blocks when the employee's finger contacted the saw blade. The employee had four fingers on one hand amputated and three fingers on the other hand amputated. The employee was hospitalized.
