An employee was attempting to repair a chain conveyor that pulls fence posts from one area of the facility to another. The employee used the emergency stop to shut the conveyor down. The machine wasn't locked out and tagged. At some point another employee turned the conveyor back on while the maintenance employee still had his hand on the conveyor chain. The employee lost his whole middle finger and the tip of his left pointer finger.
