On 08/16/2015, an employee was installing highway drainage pipes inside an excavation. A slab of asphalt that was next to the pipe trench broke loose and slid into the excavation, catching his fingers between the slab and the trench bottom. His left index finger at the knuckle and left thumb at the first joint were fully amputated at the hospital.
