An employee was driving a forklift at the Walmart Distribution Center in Smyrna, Delaware. He dismounted the moving forklift, which drove toward a vertical I-beam. His left foot got caught between the I-beam and the forklift and his toes were crushed. All of his left-foot toes were surgically amputated.
