An employee was in training on a new line when a piece of wood flooring came loose under the vacuum lift. The machine was turned off, but remained energized. The employee climbed over the fence around the vacuum lift, tripping a sensor, and the vacuum lift came down on him with wood flooring attached. The employee suffered fractured back bones and broken ribs.
