Final narrative
An employee was operating a stand up forklift travelling east bound, forks trailing, behind another stand up forklift. Forklift 2 began to turn left into a staging lane to deliver parts. The employee attempted to pass Forklift 2 on the right side. As he was passing, he saw an oncoming forklift (3) travelling west. He made a quick right-hand turn to avoid the westbound forklift (3) into aisle 45 and struck a protective barricade that attached to the concrete that protected the barrier. The employee's left foot was pinched between the barricade and the back of his stand up forklift resulting in severe soft tissue damage and multiple leg fractures. The left leg was amputated below the knee.
