Final narrative
On November 5, 2015, an employee was attempting to remove the lift hydraulic cylinder from the mast of a forklift. The employee had blocked the mast to prepare to remove the cylinder. The employee used a pry bar because the cylinder was stuck in its mounting points. While using the pry bar, both the blocking for the mast and the cylinder came out, causing the mast to crash down and smash the employee's left index finger between the mast and the frame. The finger was amputated, requiring hospitalization and stitches.
