An employee was setting up a lift with a beam clamp on a beam with a chain attached. The clamp had a hammerlock that connected the chain and beam lock. The employee's left index finger was between the chain and hammerlock when he hoisted the load. His left index finger was pinched and amputated between the hammerlock and chain when the chain tightened.
