Final narrative
An employee was vertically lowering tubes (16-inch-diameter x 12-feet-long, with a flange on top; approximate weight= 1500 pounds) into an exchanger with an overhead crane. Due to slack in the lifting attachment, the employee grabbed the flange to try to turn the tube and align it with the lower tube sheet. His fingers were pinched between that tube and the adjacent tube, amputating the top joint of his right middle finger.
